Without "to be", the sentence means that honesty is essential if there is already good rapport between the patient and the therapist.

2: Honesty is essential if there is to be good rapport between patient and therapist.

With "to be", it means that honesty is essential if the goal is to have good rapport between the patient and the therapist.
